New Clients

If you are a new client, please complete the following forms and bring them to your first therapy session. (Note: If you don’t have access to a printer, please come 15 minutes prior to the scheduled appointment and fill out these forms in my office). 

24-Hour Cancellation Policy

For your convenience, appointments are scheduled in advance. If you are unable to keep your scheduled appointment, please cancel at least 24 hours in advance. 

You will be charged one-half of the session
for cancellations within 24 hours of your appointment or for missed appointments.
In the event of a cancellation, please be sure to confirm or reschedule your next
appointment.

Financial Policy

Each session is 55 minutes in length. The charge for each session is $150.00. The financial agreement of this practice is payment at the time of services. Fees are payable to Dr. Abercrombie as an independent provider. 

I can help you file your insurance as an out-of-network provider. You are responsible for all charges not covered by insurance. Pay by credit card, cash, check, health savings account, or other convenient methods.

Confidentiality

Tennessee law states: “The confidential relations and communications between licensed psychologist and clients are placed upon the same basis as those provided by law between attorney and client (TCA 63-117)”

The client controls information regarding services. There are two exceptions to this rule: First, in the event of an emergency where there is imminent danger to the client or other persons, the psychologist may breach the requirement of confidentiality.

Second, in cases of child abuse, psychologists are required to protect the child from ongoing abuse and must breach confidentiality, if necessary, to do so. Tennessee law requires that child abuse must be reported to the Department of Human Services.

Dr. Abercrombie’s practice is compliant with the Health Insurance Portability and Accountability Act (HIPPA) which include patient protections surrounding the use of protected health information.
